New Features
- Added the ability to replace a value during invalidation.

When invalidating a program session, we are now able to replace the invalidated value with a corrected value. There is still the ability to just completely invalidate the session as before.

- Added the Hide Connect flag to the Program Configurations admin page for the COPD, Heart Failure, Diabetes, and Hypertension programs. When turned on patients are given fewer opportunities to connect with the provider contact associated with their program. This includes removing options when calling into these programs as well as modifying messaging to not highlight ways to connect with the provider contact.
- Added a flag to the Heart Failure program configuration that allows a user to turn off the weight question completely.
- Added a flag to the Heart Failure program configuration that allows a user to toggle on and off the KCCQ question.
- Added a new program for our post discharge use case called High-Touch Post Discharge. This program adds more frequent checkins right after enrollment with more opportunities for the patient to get in touch with the care team associated with their program.
Improvements
- It is now possible to see a patient's discharge date and program start date when looking at a patient's Post Discharge prescription settings. It is also possible to specify both a start date and a discharge date while enrolling a patient via the CareSignal dashboard. Before it was only possible to specify a discharge date.
Code Corrections
- Fixed an issue that would cause the SDoH program data to not display correctly in the CareSignal dashboard.
- Removed the Positive Comment column from the Quality Survey data table on a patient's profile given that we no longer ask the positive comment question.
