- A new proactive outreach pipeline is now available to help teams automatically identify unengaged patients and trigger follow-up outreach based on account-level settings.
Improvements
- It is now possible to enroll patients in the PHQ-2 to PHQ-9 workflow directly from the Depression program. Instead of always administering the full 9-question PHQ-9 survey, a CareSignal user can select "PHQ-2 → PHQ-9 (Screening)" when prescribing — patients are first asked the two-question PHQ-2, and only escalated to the full PHQ-9 if their score indicates further evaluation is needed (score ≥ 3). The patient profile now shows dedicated PHQ-2 and PHQ-2 → PHQ-9 response tables when screening mode is active, giving care teams clear visibility into screening results and escalation outcomes.
- Improved the information provided in the "upcoming communication pause" banner to make it easier for users to understand exactly what days are included in the pause.
- To align better with our other programs and to reduce texting fatigue, we have removed the secondary text that was sent out if a patient didn't respond to their first program session text, so now the following programs only send out one text:
- Substance Use
- Heart Failure
- Post Discharge
- COPD
- Removed the "Please listen carefully as our menu options have recently changed." from the Hypertension and Diabetes call in verbiage to reduce the amount that a patient has to listen to when they call in. Our menu options haven't changed for a long time, so it's no longer necessary.
- Updated Post Discharge program messaging from "We'll check in for next few weeks" to "We'll check in for the next few weeks" and wrote out the word "messages" and "text" instead of using "msg" and "txt".
- Updated the Patient archive pop up to better reflect how archiving works. Previously, "Archive" was defined as "will cause all of the associated active programs to be paused." The true definition is that archiving a patient will actually terminate the program schedule, which will go into effect 1 day after the archive is completed. If a patient is "unarchived" then the patient will need to be re-enrolled or re-prescribed into a program to restart any outbound outreach.

Expanded on the data displayed in the patient card on the CareSignal dashboard. Before, for our Hypertension program, we would display something like “Systolic max above red limit”, but now we include the value as well, so we display: “Systolic max of 189 above red limit”. This update surfaces values for our Hypertension and Diabetes programs. The following items have been adjusted to include a value:
