The Patient Details page is where you can access all of the information about a patient. This includes basic patient information, program data and trends, event notes, and individual program enrollment information.

Why It's Important

Patient Details is a one-stop shop to view and manage the self-reported data that CareSignal has collected. This is where almost all patient-related actions will be conducted after a patient has been enrolled in a CareSignal program.

From the dashboard by clicking on the patient’s name or their program
